Abstract The case of a 59-year-old female patient presenting as surgical emergency with abdominal pain and positive Obraztsova's sign. Seven days prior to the presentation, she had attended her General Practice (GP) complaining sudden onset severe occipital headache associated photophobia, vertigo, paraesthesia. Due family history cerebral aneurysms, GP urgently transferred an department, where was subsequently admitted under acute medical team. A lumbar puncture (LP) been performed exclude subarachnoid haemorrhage. investigated Computerised Tomography abdomen pelvis contrast found have arterial bleed causing haemodynamic instability psoas haematoma. An artery embolisation procedure by interventional radiology department in order stabilise bleed. made full recovery discharge. This rare exemplifies value obtaining adequate arriving at diagnosis. In appropriate clinical context, contrast-enhanced CT scan should be obtained this treatable condition. Also, measures minimize risk complications make safer incorporated into guidelines, including methods ensure that correct position L3/L4 or L4/L5 is performing procedure, use atraumatic spinal needles ultrasound guidance.
